D'Youville University

D'Youville University student-athletes are required to provide annual medical physicals and insurance information that includes coverage for athletically-related injuries. This is a pre-requisite for practice and competition. No student will be allowed to participate, in any way, until such evidence of current insurance coverage and medical information is on file with the D'Youville University Department of Athletics.

Student-athletes can fill out paperwork through the ARMS website. The attached document below will give you detailed instructions on how to complete medical forms to be eligible to participate in athletics at D'Youville for both new and returning student-athletes.

Along with medical forms, ALL student-athletes must have an up-to-date yearly physical. Please use the attached physical form we provide. We ask that you use our physical form because you must have a SPORTS PHYSICAL that states you are cleared to participate in intercollegiate athletics. ALL OTHER PHYSICALS WILL NOT BE ACCEPTED! You must send a hard copy of the physical to the Sports Medicine Staff by fax, mail, or email.

If you cannot receive your yearly sports physical until after the due ate for medical forms because of insurance reasons, please do one of the following:
    - If you are a returning student-athlete, remind the sports medicine staff that you have an up-to-date physical on file from last year. Remember to update your physical by the expiration date of your existing physical.
    - If you are a new student-athlete, please provide us with a current physical copy that you have had within one year and make plans for providing us with up-to-date physical by the expiration date of your existing physical.

*Student-athlete sport physical are to be sent to the athletic training department, NOT THE HEALTH CENTER. The health center has their own requirements that differ from the requirements we have for student-athletes.*
